Job Title: Support Coordinator (QP)

Function/Purpose/Objective

Support Coordinators will provide support, training, and supervision (including staffing and scheduling)

to the paraprofessionals.

Support Coordinators will monitor the services provided by paraprofessionals and ensure services are

delivered that reflect the philosophy of Kaleo Supports. Support Coordinators will provide

face-to-face supervision and hands-on training for paraprofessionals. Support Coordinators will assist

paraprofessionals with ideas to implement goals as written in the support plan and provide quality care.

Support Coordinators will write short-term goals and oversee the implementation of the plan.

The function of the Support Coordinator is to supervise paraprofessionals in compliance with T10A:

27G.0200 of the APSM 30-1 Manual. He/she will evaluate and ensure employees demonstrate core

skills including technical knowledge, cultural awareness, analytical skills, decision-making,

interpersonal skills, communication skills, and clinical skills. He/she will complete assessments

according to Kaleo Supports, Inc. prior to delivering services to include the person’s presenting problem,

needs & strengths, admitting diagnosis, social, family, and medical history, evaluations or assessments,

such as psychiatric, substance abuse, medical and vocational as appropriate to the person’s need.

Primary Duties

Recruit, interview and hire employees in accordance to state requirements.

Provide training to paraprofessionals and associate professionals.

Implement supervision plans and update as defined and complete employee performance reviews in

accordance with policy.

Review paraprofessionals’ and associate professionals’ documentation.

Participate in individual support plan meetings, write short-term goals, and develop strategies for goal

implementation.

Evaluate documentation for progress toward goals in accordance with each Individual Support Plan and

complete appropriate reports.

Create service records for new admissions including assessments, consents, acknowledgement of rights,

HIPPA, etc.

Secure required documentation necessary for service records.

Keep service records current including progress notes.

Work in partnership with case managers, family members, and other team members to ensure effective

implementation of services.

Continuing responsibilities/expectations/accountability

Effective communication with people, other employees, and stakeholders

Report incidents including alleged or witnessed abuse, neglect, exploitation

Oversee the implementation of company policy and procedures

Develop positive relationships with people supported by the company, family members, and others

Follow state and federal rules

Implement best practices and the philosophy of Kaleo Supports

Represent the company within the community

Roles and Relationships

A Support Coordinator will supervise other employees as assigned. His or her supervisor is the Director

of Supports and/or the Executive Director. He/she will oversee the employees of the company with the

philosophy that all employees are representative of the quality of services provided by Kaleo Supports,

Inc.

Minimum Requirements and Supervision

A Support Coordinator must be a qualified professional as defined in 10A NCAC 27G.0100-0200.

Qualified professionals as specified in 10A NCAC 27G.0104 (18) (a) are deemed to have met the

requirements of the competency-based employment system in the State Plan for MH/DD/SA.

"Qualified professional" means, within the mh/dd/sas system of care:

(a) an individual who holds a license, provisional license, certificate, registration or permit issued by the governing board

regulating a human service profession, except a registered nurse who is licensed to practice in the State of North Carolina by

the North Carolina Board of Nursing who also has four years of full-time accumulated experience in mh/dd/sa with the population

served; or

(b) a graduate of a college or university with a Masters degree in a human service field and has one

year of full-time, post-graduate degree accumulated mh/dd/sa experience with the population

served, or a substance abuse professional who has one year of full-time, post-graduate degree

accumulated supervised experience in alcoholism and drug abuse counseling; or

(c) a graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, post-bachelors

degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has two

years of full-time, post-bachelor degree accumulated supervised experience in alcoholism and drug abuse counseling; or

(d) a graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time,

post-bachelors degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional

who has four years of full-time, post-bachelors degree accumulated supervised experience in alcoholism and drug abuse

counseling

Skills Required

 Ability to communicate professionally and effectively with various people (individuals with

 To look beyond pre-conceived assumptions about people with developmental disabilities and

mental retardation that are limitations because of the disability

 Ability to discipline and separate own emotions from those of the person supporting and/or

family

 Ability to understand and accept that compliance is institutional, and helping people achieve

dreams and goals require meaningful supports

 Self-directed, ability to work independently and without daily supervision

 Maintain required documentation in a timely manner and be very organized

 Ability to supervise and manage employees
